It is important that you participate in planning your health care with your physicians and nurses. Our brochure "Speak Up, Ask Questions!" explains what you can do as the patient or family advocate in making health care safe:

• Ask a question when you do not understand what is going on.
• Make sure your physician knows what medications you are currently taking. This includes prescription and over the counter medications, and dietary supplements such as vitamins or herbs.
• Make sure healthcare providers and visitors wash their hands.
• Ensure proper identification. Expect health care workers to introduce themselves; look for their identification badges.
• Make sure your health care professional confirms your identify when you are going to a test or treatment, receive any medication or if you receive blood or blood products.
• If you are scheduled for surgery, make sure you are asked to confirm where (on what part of the body) you are having surgery; and the surgery site will be marked by your doctor if applicable.

Process of Care Measures

Process of care measures are a reflection of the care provided to patients.
